I was wondering what everyone does with regards to domain registration for clients?
I use www.123-reg.co.uk as they have proved to be cheap and have a great Control Panel, along with friendly customer support.
Is it misleading to register domains for clients and hosting clients without telling them who I am actually registering with?

Personally, I don't think it's misleading. As long as you tell them that you don't register the domain, but someone else does, then it's ok I suppose. There really aren't many features that you need when managing a domain. The only thing that I have really bothered with are the name servers.
I've got domains all over the place. I've got one at Go Daddy/Cyber Wings, one with Terra-Byte.com and that's about it. Ok, that's not that bad, but you get the point.

What I plan to do is make them a Control Panel for their domain and when they update it will automatically email me then I can make the changes for them.
Many clients don't even know how to register a domain and some don't even have a credit card, as I accept cash, cheque (check) that gives me an advantage.
Any whois lookup still shows the domain registrar is hosteurope. I know OpenSRS and dotster do domain reseller programs but right now I don't have the time to set those up and OpenSRS is a bit of a nightmare to setup I believe.

Heck, a lot of people are shocked when you tell them, "You have to buy a domain, too". They'll just greet you with the "I already have one, right?" answer. They don't know the difference between a domain and server space.

Well we register domain names as well as we are an OpenSRS reseller. We register domains in the clients name always, never in our name. OpenSRS has an API for managing the domain that we use. OpenSRS is a great place to resell for, and we highly recommend them. From what I hear enom is also a good place and they are cheaper than opensrs.
